Become a Member

Prospective members must submit the following to become a member:

1. Complete the Intake Form

2. Complete the Physician Clearance Form

3. Read & sign the Code of Conduct Form

4. Read & sign the Waiver of Liability Form

5. Provide proof of age (all members must be 60+ years of age)

6. Provide proof of DC residency (utility bill or other document listing current address)

Hard copies of these documents may be picked up for completion or submitted for approval during Walk-in Hours only: Tuesdays and Thursdays only from 9am to 3pm.

During Walk-in ours, prospective members can get a tour of the facility, meet with a staff member and pick-up/submit documents.
